Cancellation, Deposit, Mobile Service, Grace Period & No‑Show Policy

1. Deposit Requirement (In‑Studio Services)

A $50 deposit is required at the time of booking to reserve any in‑studio appointment.

  • The deposit is applied toward the final cost of the session.

  • Deposits are non‑refundable unless the appointment is canceled at least 24 hours in advance.

2. Mobile Massage Payment Policy

Mobile massage is a premium, by‑appointment‑only service.

2.1 Full Payment Required at Booking

Clients must pay the full service price at the time of booking. This payment is non‑refundable if the cancellation occurs within the 24‑hour window.

2.2 Mobile Service Fee

All mobile sessions include a $45 luxury mobile service fee, added to the posted single‑session rate. This fee covers travel, setup, equipment transport, and premium on‑location service.

2.3 By Appointment Only

Mobile services are offered by appointment only and subject to therapist availability, travel radius, and safety considerations.

3. 24‑Hour Cancellation Policy

Cancellations must be made at least 24 hours before the scheduled appointment time.

3.1 In‑Studio Sessions

  • Cancellations made before the 24‑hour window may transfer the deposit to a future session.

  • Cancellations made within 24 hours result in forfeiture of the $50 deposit.

3.2 Mobile Sessions

  • Cancellations made before the 24‑hour window may transfer the full prepayment to a future mobile session.

  • Cancellations made within 24 hours result in forfeiture of the full service price, including the $45 mobile fee.

4. No‑Call / No‑Show Policy

A no‑call/no‑show occurs when a client fails to arrive and fails to contact FUNCTION FORMED prior to the appointment start time.

4.1 In‑Studio Sessions

A no‑call/no‑show results in:

  • Forfeiture of the full appointment price

  • Loss of the $50 deposit

  • Mandatory prepayment for all future bookings

4.2 Mobile Sessions

A no‑call/no‑show results in:

  • Forfeiture of the full prepaid amount

  • Possible restriction of mobile service access

5. Rescheduling

Rescheduling follows the same rules as cancellations:

  • Rescheduling 24+ hours in advance is allowed without penalty.

  • Rescheduling within 24 hours is treated as a late cancellation.

6. Mobile Service Requirements

Clients booking mobile services must provide and maintain a safe, appropriate environment for treatment. This includes:

  • A clean, temperature‑controlled space with adequate room for the massage table

  • Secured pets

  • Reasonable parking access

  • No active smoking in the treatment area

  • No individuals present who are under the influence of drugs or alcohol

6.1 Required Safety Information

For safety and security, clients must provide the names and phone numbers of any additional adults who will be present in the home during the session. This information must be submitted at the time of booking or prior to the appointment. Failure to provide this information may result in cancellation of the session and may be treated as a late cancellation.

6.2 Therapist Discretion

If the environment is unsafe, unpredictable, or does not meet the above requirements, the therapist may refuse or terminate the session. Such situations may be treated as a late cancellation, and applicable fees will apply.

7. Grace Period

A 15‑minute grace period is provided solely to prevent immediate lock‑out. The grace period does not extend the session.

  • Time booked = time reserved.

  • The session will end at the originally scheduled end time.

  • The therapist is not obligated to work past the reserved time and may decline to extend the session at their discretion.

If the client has not arrived or made contact within 15 minutes of the scheduled start time, the appointment is considered a no‑call/no‑show, and the full appointment price (including any mobile service fees) is forfeited.

8. Therapist Rights

FUNCTION FORMED reserves the right to:

  • Refuse service for repeated violations

  • Require full prepayment for clients with prior late cancellations or no‑shows

  • Terminate a session early if behavior is inappropriate or unsafe (full payment still applies)

9. Agreement

By booking an appointment, the client acknowledges and agrees to all terms listed above.

10. Professional Code of Conduct

FUNCTION FORMED maintains a strict professional standard to ensure the safety, comfort, and ethical treatment of both clients and the therapist. All clients are expected to behave respectfully and appropriately at all times.

10.1 Zero‑Tolerance Policy for Sexual Misconduct

Any form of sexual misconduct will result in immediate termination of the session without refund, including but not limited to:

  • Sexual comments, requests, or propositions

  • Lewd or suggestive behavior

  • Exposure of genitals

  • Attempting to touch the therapist inappropriately

  • Any action intended to initiate sexual contact

This policy applies equally to:

  • In‑studio sessions

  • Mobile sessions

In cases of severe misconduct, FUNCTION FORMED reserves the right to:

  • Permanently ban the client from future services

  • Contact law enforcement if necessary

  • Document and report the incident for safety purposes

10.2 Respectful Conduct

Clients must:

  • Communicate clearly and respectfully

  • Follow therapist instructions related to safety and positioning

  • Maintain appropriate hygiene

  • Refrain from aggressive, threatening, or disruptive behavior

10.3 Therapist Rights

The therapist may terminate a session immediately if:

  • The client’s behavior becomes inappropriate, unsafe, or violates this Code of Conduct

  • The environment (for mobile sessions) becomes unsafe or unpredictable

Full payment still applies for any session terminated due to client misconduct.
